Social Worker

Duties

This is highly independent professional social work, providing a wide range of services to members who have a variety of problems related to disability and aging. The primary goal is to provide casework counseling services including:  assessment, care planning; implementation of intervention strategies; advocacy; admissions; and discharge services. Work is performed in coordination with a multi-disciplinary team to enhance the quality of services offered and promote the understanding of the aging/disabled to families, other staff, and the community.

Required Technical Knowledge, Skills and Abilities

  • Federal and state regulations applicable to a long-term health care facility
  • Social work principles including techniques of interviewing and counseling methods specific to geriatric residents
  • Occupational safety standards
  • Operation of computerized information systems
  • Ability to interact with health care professional in developing integrated, individualized care plans
  • Interpretative skills to communicate with members/families in making assessment
  • Skill in assessment, development, and implementation of interventions to meet the psychosocial needs of geriatric residents including those with behavior problems
  • Effective communication

Special Requirements

Licensure as a Certified Social Worker or higher with the State of Wisconsin and, to comply with federal nursing home regulations, one (1) year of supervised social work experience in a setting working directly with individuals subject to health issues.
