As part of the "I Owe You" outreach campaign to help ensure Wisconsin's veterans are receiving all the benefits they have earned, Supermarkets of Veterans Benefits were created to bring together in one location those federal, state, and local agencies that provide services to veterans and their families.

If you belong to an organization that provides services to veterans, and you would like to set up a booth at the next Supermarket of Veterans Benefits, complete and submit form WDVA 0150, "Supermarket of Veterans Benefits – Request for Provider Space."
Veterans applying for benefits at the Supermarkets of Veterans Benefits should bring a copy of their discharge form (DD Form 214). Information on how to obtain a DD Form 214 is available here.
